Deductibles  
Deductibles are subject to collection at time of visit.

Co-Payments
Co-Payments are collected at the time of service.

Self Pay
Patients that do not have insurance will be required to pay a cash deposit of $350.00 prior to their appointment.  Any remaining balance from that visit will be collected at check out or if there has been an overpayment the refund will be given at that time.

Disability Insurance Forms
There is a prepayment form completion fee of $5.00 per form.  Please allow 5-7 days for the completion of your form from the day of receipt.

Medical Records
Your medical records are available to you at no charge upon receipt of your signed release within 3-5 days of request.
